When hair loss is caused by a medical condition or its treatment, a wig is often documented as a cranial prosthesis. Ask your insurance company whether your plan covers a cranial prosthesis and what documentation it requires.

1. Ask your insurer whether a prescription is required and what wording your doctor should use.
2. Call the member number on your insurance card and ask specifically about cranial prosthesis benefits, annual limits, and whether pre-authorization is required.
3. Ask whether they reimburse you directly, and what documentation they need.
4. Bring what you learn to your appointment. We'll provide an itemized receipt and appropriate documentation for your claim.
